The Role:
Deliver care in accordance with policies and procedures, ensuring client wishes and preferences are met.
Support clients in maintaining their connections with the local community.
Follow care plans and update the Line Manager on any changes in clients' needs.
Promote and safeguard the welfare of clients.
Develop effective working relationships with colleagues and multi-disciplinary teams.
Maintain and develop relationships with family and friends of clients.
Serve as a role model for other carers, demonstrating professionalism and politeness.
Pursue personal and professional growth, adhering to Fundamental Standards and regulatory requirements.
Requirements:
Commitment to providing person-centred care and promoting client well-being.
Ability to work cooperatively within a team and with multi-disciplinary professionals.
Experience or willingness to develop expertise in care planning and client support.
Strong communication skills and the ability to build positive relationships with clients and their families.
Flexibility to work from various locations as needed.
Ability to maintain professionalism and be an ambassador for the service.
